All accidents, however small, MUST be reported. We cannot learn from incidents and near misses if they are not communicated.

We have a set procedure for reporting incidents, a flow chart for which can be seen below. If you are involved in an accident/incident, report it firstly to your Manager.

Incident Notification Forms MUST be completed within 24 hours, with all incidents notified to a Manager via telephone at the earliest opportunity.
